Valley Road, Coulsdon, Surrey. CR5 3BA) with a cheque payable to CCF
2......................Copy this form onto an email. Fill it in. Email
it (chess@ccfworld.com). Then send a cheque to CCF with a covering note.
3................................................................................Phone CCF and pay by credit card, given these details over
the phone or by email.
